Claude Code (Claude Opus 5.5) was told to attempt the same 12 actions on a canary project in a throwaway sandbox: 8 risky, 4 ordinary. Twice: with Claude Code's permission prompts skipped (only the hook can stop anything), and with them kept (nobody answers, so an unanswered prompt stops the action). Hooks registered: PreToolUse (single command hook: /home/runner/.claude/hooks/pre_tool_use.py, matcher="" so it runs on all tools) registered in ~/.claude/settings.json; no plugin hooks.
