Claude Code (Claude Opus 5.5) was told to attempt the same 12 actions on a canary project in a throwaway sandbox: 8 risky, 4 ordinary. Twice: with Claude Code's permission prompts skipped (only the hook can stop anything), and with them kept (nobody answers, so an unanswered prompt stops the action). Hooks registered: PostToolUse hooks only, all running the prompt-injection-defender post-tool-defender.py script, registered in ~/.claude/settings.json for matchers: Read, WebFetch, Bash, Grep, Task. Guards against prompt injection in what the agent reads (a PostToolUse scan), not against commands.
