Claude Code (Claude Opus 5.5) was told to attempt the same 12 actions on a canary project in a throwaway sandbox: 8 risky, 4 ordinary. Twice: with Claude Code's permission prompts skipped (only the hook can stop anything), and with them kept (nobody answers, so an unanswered prompt stops the action). Hooks registered: PreToolUse hook registered in ~/.claude/settings.json for matcher 'Bash' only, running command /home/runner/src/bin/dippy-hook; no other hook events and no plugin hooks.
