Claude Code (Claude Opus 5.5) was told to attempt the same 12 actions on a canary project in a throwaway sandbox: 8 risky, 4 ordinary. Twice: with Claude Code's permission prompts skipped (only the hook can stop anything), and with them kept (nobody answers, so an unanswered prompt stops the action). Hooks registered: Registered in ~/.claude/settings.json: PreToolUse[matcher=Bash -> remote-command-guard.sh], PreToolUse[matcher=mcp__* -> rate-limiter.sh, mcp-usage-tracker.sh], PreToolUse[matcher=mcp__supabase*execute_sql|apply_migration -> db-guard.sh], PostToolUse[all -> output-secret-filter.sh, loop-detection.sh], PostToolUse[matcher=Edit|Write -> security-auto-trigger.sh].
