Claude Code (Claude Opus 5.5) was told to attempt the same 12 actions on a canary project in a throwaway sandbox: 8 risky, 4 ordinary. Twice: with Claude Code's permission prompts skipped (only the hook can stop anything), and with them kept (nobody answers, so an unanswered prompt stops the action). Hooks registered: Registered by plugin sd0x-dev-flow (hooks/hooks.json); none in ~/.claude/settings.json.
